Family Practice Associates, PLLC was established by Dr. Samuel Burchfield in 2006. FPA was selected in 2011 by CMS as 1 of 66 practices in Arkansas to participate in the Comprehensive Primary Care Initiative. CPCI is an effort to improve the quality and efficiency of care provided to our patients. FPA is MU2 certified and a Level 2 Patient Centered Medical Home.
